Amaç: Bu çalışmanın amacı egzersiz ile kombine gece ortezi ve bantlama uygulamalarının halluks valgus (HV) deformitesi, ağrı şiddeti ve yaşam kalitesi üzerine etkilerini karşılaştırmaktı. Yöntem: Çalışmaya 10 kadın hastanın 17 (bilateral:7, unilateral:3) ayağı dahil edildi. Bireyler, ortez grubu ve bantlama grubu olarak ikiye ayrıldı. Ortez grubuna HV gece ortezi, bantlama grubuna ise rijit bant 8 hafta süreyle uygulandı. Ayrıca bütün katılımcılar tarafından günde bir kez, 15 tekrarla, toplam sekiz hafta süreyle HV egzersizleri uygulandı. Bireylerin halluks valgus açıları (HVA) ve intermetatarsal açıları (IMA) radyografik yöntem ile başlangıçta ve tedaviden hemen sonra değerlendirildi. Çalışmada kullanılan diğer değerlendirme yöntemleri ise Ayak Fonksiyon İndeksi (AFİ) ve Manchester-Oxford Ayak Anketi (MOXFQ) idi. Gruplar arası fark Mann Whitney U testi ile analiz edildi. Anlamlılık düzeyi p<0,05 olarak belirlendi. Bulgular: Grupların tedavi öncesi ile tedavi sonrası ortalama değerleri arasındaki farklar karşılaştırıldığında HVA’nda bantlama grubu lehine anlamlı fark olduğu gözlendi (p<0,05). MOXFQ yürüme ve ayakta durma alt skalası sonuçları ise ortez grubu lehine anlamlı fark olduğu gözlendi (p<0,05). Sonuç: Çalışmamızın sonuçları, bantlama uygulamasının deformitenin azaltılmasında daha etkili olduğunu, gece ortezinin ise yürüme ve ayakta durma fonksiyonunu iyileştirmede daha etkili olduğunu gösterdi. Bu sonuçlar bize ortez ve bantlama uygulamalarının HV tedavisinin farklı parametrelerinde birbirlerine göre üstün olduğunu göstermektedir. Klinikte hasta yararı gözetilerek uygun tedavi yöntemi tercih edilmelidir.

Comparison of the effects of night orthesis and bandage methods combined with exercise in women with hallucus valve: a pilot study

Purpose: The aim of this study was to compare the effects of night orthesis and bandage practices combined with exercise on hallux valgus (HV) deformity, pain severity and quality of life. The study included 17 (bilateral:7, unilateral:3) female patients. The individuals were divided into two as a group of orthes and a banding group. Ortez group was applied to HV night ortez, and to the banding group the riit band was applied for 8 weeks. All participants also performed HV exercises once a day, 15 repetitions, for a total of eight weeks. The hallucus valgus angles (HVA) and intermetatarsal angles (IMA) of individuals were evaluated by radiographic method at the beginning and immediately after treatment. Other methods used in the study were the Foot Function Index (AFI) and the Manchester-Oxford Foot Survey (MOXFQ). The differences between the groups were analyzed by the Mann Whitney U test. The level of significance was determined as p<0,05. Results: When compared the differences between the average values of groups before treatment and after treatment, a significant difference was observed in the HVA in favour of the banding group (p<0,05). The MOXFQ foot and standing scale resulted in significant differences in favor of the orthesis group (p<0,05). Results: The results of our study showed that the banding application was more effective in reducing deformity, while the night orthesis was more effective in improving the function of walking and standing. These results show us that the orthesis and bandage applications are superior to each other in the different parameters of HV treatment. In the clinic, the patient’s benefits should be monitored and the appropriate treatment method should be preferred.
