Tirotoksik periyodik paralizi total vücut potasyumu eksikliği olmaksızın; akut hipokalemi, kas güçsüzlüğü ve tiroksoksikoz triadı ile karakterize bir hastalıktır. Olguda tirotoksik periyodik paralizi ile ilişkili bir vaka sunulmuştur. Afgan uyruklu 21 yaşında erkek hasta ani gelişen halsizlik, kas güçsüzlüğü şikayetleri ile acil servise başvurdu. Nörolojik muayenede alt ekstremite motor kas gücü proksimal 2/5, distal 4/5, üst ekstremite kas gücü proksimal 3/5, distal 4/5 saptandı. Potasyum düzeyinin 2.6 mmol/L ve semptomatik olması üzerine hastaya intravenöz (IV) KCL replasmanı başlanarak takibe alındı. Hipopotasemi ayırıcı tanısına yönelik bakılan tiroid fonksiyon testlerinde aşikar hipertiroidi (TSH: 0.01 IU/Ml sT4: 4.32 ng/dL sT3: 17.4 pg/mL) saptandı. EKG de uzamış QT, yaygın ST depresyonu, patolojik U dalgası mevcuttu. Semptomatik olan hastaya IV ve oral potasyum replasmanı uygulanırken 1., 4. ve 12. saat potasyum ölçümleri yapıldı. 4. saat potasyum düzeyinin istenilen seviyede olması ve hastanın semptomlarında dramatik düzelme olması üzerine potasyum infüzyonu stoplandı. Hipertroidi için hastaya metimazole 45 mg/gün ve propranolol 40 mg/gün tedavisi başlandı. Tirotoksik periodic paralizi nadir görülen bir hastalıktır ancak tedavi edilmediği takdirde ölümcül olabilir.
Periodic thyrotoxic paralysis is a disease characterized by acute hypokalemia, muscle weakness and thyroxoxic triade. There is a case associated with periodic thyrotoxic paralysis. A 21-year-old Afghan man with a sudden developing illness, muscle weakness complaints and applied to emergency service. In the neurological examination, the lower end motor muscle strength proximal 2/5, distal 4/5, the upper end muscle strength proximal 3/5, distal 4/5 was identified. After the potassium level was 2.6 mmol/L and symptomatic, the patient was followed by the start of intravenous (IV) KCL replacement. In thyroid function tests considered for the differentiating diagnosis of hypothesia, obvious hyperthyroidism (TSH: 0.01 IU/Ml sT4: 4.32 ng/dL sT3: 17.4 pg/mL) was detected. ECG also had extended QT, common ST depression, pathological U wave. Symptomatic patients were given potassium replacement IV and oral, while potassium measurements were made at 1, 4 and 12 hours. The 4-hour potassium infusion was stopped after the potassium levels were at the desired level and the patient's symptoms had a dramatic improvement. The patient received 45 mg/day metimazole and 40 mg/day propranolole. Tyrotoxic periodic paralysis is a rare disease, but it can be fatal if not treated.
