Mastit ve Meme Absesi Tanılı Yenidoğan Vakalarımızın Değerlendirilmesi

Abstract:

Amaç: Yenidoğanlarda nadir görülen mastit ve meme absesi, meme hipertrofisi olan bebeklerin meme dokusuna masaj gibi elle müdahalelerle veya nadiren hematojen yayılımla oluşur. Mastit antibiyotik tedavisi ile düzeltilebilirken, abseye cerrahi drenaj gerekebilir. Çalışmamızda yenidoğan mastiti ya da meme absesi tanısı alan hastaların incelenmesi planlandı. Gereç ve Yöntem: 2012 - 2018 yıllarında yenidoğan (YD) biriminde yatan hastaların dosyalarındaki tanıları incelendi. Mastit ya da meme absesi tanılı hastalar tespit edildi. Klinik bulguları, başvuru şikayetleri, tam kan sayımı (CBC), C - reaktif protein (CRP) kan kültürü, yara kültürü sonuçları, verilen tedavi ve hastanede yatış süreleri kaydedildi. Yenidoğan mastiti (YDM) yüzdesi hesaplandı. Bulgular: Yirmibinkırkiki doğumdan ve yenidoğan (YD) biriminde yatan 2252 hastadan yalnızca 6'sında mastit ve meme absesi tanısı mevcut idi. Buna göre ilimiz mastit/meme absesi sıklığı %0,03; erkek/kız oranı 1/2; meme absesi / mastit oranı %50'dir. Meme absesi olan iki bebekten birinde beyaz kan hücresi (WBC) değeri 19500/mm3 olup, CRP artmış ve kan kültüründe üreme saptanmış, diğerinde yalnız CRP artmış ve drenaj kültüründe üreme saptanmış idi. Hastaların yarısına ampisilin ve gentamisin verilmiş, komplikasyon gelişmeden bebeklerin iyileştiği görülmüştür. Sonuç: Mastit ve meme absesi nadir görülen, tedavi edilmezse sonuçları kötü olabilen ciddi sağlık problemidir.

Mastitis and breast abscess assessment of our newborn cases

Abstract:

Purpose: Mastitis and breast abscess, which is rare in newborns, occurs by hand interventions such as massage to breast tissue in infants with breast hypertrophy or rarely by hematogenic spread. Mastitis can be corrected with antibiotic treatment, while the abscess may require surgical drainage. Our study was planned for the study of patients diagnosed with newborn mastitis or breast abscess. Tools and Methods: Diagnoses in the files of patients in the newborn (YD) unit were examined in the years 2012 - 2018. Patients with Mastitis or breast abscess have been identified. Clinical findings, application complaints, full blood count (CBC), C-reactive protein (CRP) blood culture, wound culture results, given treatment and hospitalization periods were recorded. The percentage of newborn mastitis (YDM) has been calculated. Results: In 2252 patients from twenty-two births and newborns (YD) only 6 were diagnosed with mastitis and breast abscess. According to this, the frequency of mastitis/mastitis is 0.03 %; the male/woman ratio is 1/2; the breast abscess/mastitis ratio is 50 %. One of the two babies with breast abscess had a white blood cell (WBC) value of 19500/mm3, CRP increased and reproduction was detected in the blood culture, the other only CRP increased and reproduction was detected in the drainage culture. Half of the patients were given ampisillin and gentamisin, and the infants were healed without developing complications. The result: Mastitis and breast abscess are a rare, serious health problem, if not treated, the results may be bad.
