Radius Alt Uç Eklem İçi Kırıklarında Artroskopi Yardımlı Osteosentez

Abstract:

Amaç: Radius alt uç eklem içi kırıklarında, anatomik eklem restorasyonu amacıyla, kırık redüksiyonu artroskopi yardımıyla fluoroskopi kontrolünde yapılarak, eşlik eden eklem içi patolojilerinin sıklığı ve şiddeti araştırılıp, prospektif olarak olguların sonuçları değerlendirildi. Yöntemler: Distal radius eklem içi kırığı olup, redüksiyon sağlanamayan veya instabil kırığı olan on iki erişkin hasta çalışmaya alındı. Eksternal fiksatör ile distraksiyon sağlandı. Eklem yüzeyi artroskopik gözlem altında restore edildikten sonra fluoroskopi kontrolünde K-telleri ile stabilize edildi ve eksternal fiksatör kilitlendi. Klinik değerlendirmelerde DASH (disabilty arm shoulder hand) anketi, Gartland&Werley skorunun Sarmiento modifikasyonu (GWSM) ve Mayo modifiye el bileği skoru (MWSM) kullanıldı. Bulgular: Altı hastada TFCC (triangular fibrokartilaj kompleksi) yırtığı (%50), beşinde SLIL (skafolunat interosseoz ligament) lezyonu (%41,6) saptandı. TFCC yırtığı olan hastaların dördünde (%66) skafolunat bağ lezyonuda vardı. Takip süresi ortalama 1,5 yıl (16-23 ay) olan hastaların son radyografilerinde radiusta dorsal açılanma, uzunluk veya radial açılanma kaybı olmadığı saptandı. DASH skoru ortalama 6,1 (0-15,4) bulundu. GWSM’na göre üç hastada iyi sonuç, dokuz hastada mükemmel sonuç saptandı. MWSM’na göre dört hastada iyi, sekiz hastada mükemmel sonuç alındı. Sonuç: Distal radius eklem içi kırıklarında osteosentez fluoroskopi kontrolünde artroskopi yardımı ile yapıldığında, radiusun eklem yüzeyinin anatomik restorasyonu sağlanabilir. Aynı zamanda eklem içi bağ yaralanmalarının ve kondral lezyonların tespit edilip, gereğinde debridman veya tamiriyle ileride oluşabilecek komplikasyonlardan artroskopisi yardımlı cerrahi ile kaçınılabilir.

Abstract:

Purpose: Radius in the lower end joint fractures, with the aim of anatomical joint restoration, the fracturized reduction was performed under fluoroscopy control with the help of arthroscopy, the frequency and severity of accompanying joint pathologies were investigated, and the results of the events were prospectively evaluated. Methods: Twelve adult patients with distal radius joint fractures, which are unable to reduce or unstable fractures, were taken to study. Distraction is provided with an external fixator. After the joint surface was restored under arthroscopic observation, it was stabilized with the K-tells under fluoroscopy control and the external fixator was locked. Clinical assessments used the DASH (disabilty arm shoulder hand) survey, the Gartland&Werley score's Sarmiento modification (GWSM) and the Mayo modified hand arm score (MWSM). Results: In six patients TFCC (triangular fibrocartilage complex) broken (50%) and in five SLIL (skafolunat interosseos ligament) injury (41.6) was detected. Four of the patients with TFCC break (66%) had skafolunat joint injury. The latest radiographs of patients with an average of 1.5 years (16-23 months) found no loss of dorsal opening, length or radial opening on the radius. The DASH score was 6.1 (0-15.4) on average. According to GWSM, good results were found in three patients, and excellent results were found in nine patients. According to MWSM, four patients were good, eight patients were excellent. Result: When distal radius is performed with the help of arthroscopy in osteosenthesis fluoroscopy control in joint fractures, the anatomical restoration of the joint surface of the radius can be achieved. It can also be prevented by arthroscopy-assisted surgery from complications that may occur in the future with the detection of joint injuries and condral injuries and, if necessary, deprivation or repair.
