Çocukluk Çağında Nadir Görülen Bir Kraniyal Sinir Tutulumu: Akut İzole Tek Taraflı Glossofaringeal Sinir Felci

İzole glossofaringeal sinir paralizisi çocukluk çağında çok nadir görülen bir durumdur. Geçmişte, bu durumda yer kaplayıcı oluşum, infeksiyon, travma, vasküler anomaliler, otoimmun hastalıklar veya iskemiye bağlı nöropatinin eşlik ettiği olgular bildirilmiştir. Biz bu makalede 11 yaşında akut başlangıçlı tek taraflı izole glossofaringeal sinir tutulumu olan bir olguyu sunmaktayız. Hasta nazone konuşma ve sıvı gıdaların nazal regürjitasyonu ile başvurdu. İnfeksiyon, yakın zamanda aşılanma veya travma öyküsü yoktu. Otorinolaringolojik muayenesinde izole glossofaringeal sinir paralizisi dışında bir bulgu gözlenmedi. Tanısal görüntüleme ile patolojik bulgu saptanmadı. Hasta geniş spektrumlu antibiyotik ve kortikosteroid uygulanarak başarı ile tedavi edildi. Çocuklar, nadiren nedeni belirlenemeyen akut başlangıçlı izole alt kraniyal sinir felci ile başvurabilirler. Kapsamlı tetkikler ile altta yatan neden belirlenemediğinde düzenlenen anti-inflamatuar tedavi ve antibiyoterapi ile kısa sürede yüz güldürücü sonuçlar alınabilir.

A Rare Case Of Cranial Nerve Involvement In Children: Isolated Glossopharyngeal Nerve Palsy

Abstract:

Isolated glossopharyngeal nerve palsy is a very rare condition in childhood. Several cases associated with space-occupying lesions, infections, trauma, vascular abnormalities, autoimmune diseases, or neuropathy due to ischemia have been reported. Here we report an 11-year-old boy with acute onset, unilateral, isolated glossopharyngeal involvement. The patient presented with rhinolali and nasopharyngeal regurgitation of liquids. He had no history of infection, recent vaccination, or trauma. Isolated glossopharyngeal nerve paralysis was the only pathological finding in his otorhinolaryngological examination. Diagnostic imaging was normal. The patient was successfully treated with broad-spectrum antibiotics and corticosteroids. Children can rarely present with acute onset isolated lower cranial nerve palsy due to inflammatory processes of unknown cause. The cranial neuropathies with unknown causes despite extensive research can be treated with anti-inflammatory therapy and antibiotics, and favorable results can be obtained in a short time.
