KLİNİĞİMİZDE ÜROLOJİ ASİSTANLARINCA UYGULANAN ESWL TEDAVİSİNİN SONUÇLARI

Abstract:

Amaç: Hastanemizin ESWL Ünitesi’nde bir aylık eğitim sonrası üroloji asistanları tarafından böbrek ve üreter taşı nedeniyle PCK Stonelith-V3 Lithotriptör cihazı kullanılarak ESWL Extracorporeal Shock Wave Litotripsi uygulanan hastaların sonuçlarının değerlendirilmesi amaçlanmıştır.Gereç ve Yöntem: Mayıs 2006 ile Haziran 2007 arasında böbrek veya üreter taşı nedeniyle ESWL uygulanan 171 hasta retrospektif olarak değerlendirildi. Ortalama yaş 50.1 yıldı. ESWL uygulanan taşların 125 tanesi böbrek taşı ve 46 tanesi üreter taşı idi. ESWL uygulanan taşların üriner sistemdeki lokalizasyonu: % 50.2 pelvis, %5.84 üst kaliks, %8.77 orta kaliks ve % 8.18’i alt kaliks taşıyken; %23.9’u üst üreter ve % 2.9’u orta üreter taşıydı. Odaklama elipsoid odaklı C kollu skopi ile yapıldı. Tüm ESWL uygulamaları 1 ay eğitim verilen üroloji asistanları tarafından Stonelith-V3 Lithotriptör kullanılarak yapıldı. Hasta başına uygulanan ortalama şok sayısı 2346 ve ortalama voltaj 18 kv idi. Her vakaya ortalama 3 1-5 arası ESWL seansı uygulandı. Bulgular: İki aylık takip sonunda pelvis, üst kaliks, orta kaliks, alt kaliks, üst üreter ve orta üreter taşlı hastalarda, taşsızlık oranı sırasıyla %80.3, %90.0, %86.7, % 78.5, %80.4 ve %100 idi. Sonuç: Verilerimiz, bir eğitim hastanesinde bir aylık eğitim sonrası ESWL tedavisinde literatürde bildirilen başarı oranlarına benzer sonuçların elde edilebileceğini göstermektedir

Purpose: One month post-training in the ESWL unit of our hospital is aimed at evaluating the results of patients applied ESWL Extracorporeal Shock Wave Litotripsi using the PCK Stonelith-V3 Lithotriptor device due to kidney and productive stone by urological assistants.Method and method: 171 patients applied ESWL due to kidney or productive stone between May 2006 and June 2007 were evaluated as retrospective. The average age was 50.1 years. 125 of the stones applied to ESWL were kidney stones and 46 were producing stones. Localization of ESWL applied stones in the urine system: 50.2 percent pelvis, 5.84 percent upper calcium, 8.77 percent middle calcium and 8.18 percent lower calcium; 23.9 percent upper production and 2.9 percent middle production stones. The focus was made with elipsoid-oriented C-hand scope. All ESWL applications were performed using Stonelith-V3 Lithotriptor by 1 month trained urology assistants. The average number of shocks applied per patient was 2346 and the average voltage was 18 kv. For each case, an average of 3 1-5 ESWL sessions were applied. Results: At the end of two months of follow-up, in patients with pelvis, upper calcium, middle calcium, lower calcium, upper producer and middle producer stones, the stone-free rate was 80.3, 90.0, 86.7, 78.5, 80.4 and 100 percent respectively. Result: Our data shows that results can be achieved similar to the success rates in the literature in ESWL treatment after a month of training in an educational hospital.
