Hirsutizmi olan kadınlarda tanı, insülin direnci ve beden kitle indeksi arasındaki ilişki

Abstract:

Amaç: Bu araştırmada, hirsutizm yakınması olan kadınlarda Ferriman-Gallwey (FG) Skoru ile tanı grupları, insülin direnci ve beden kitle indeksi arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Tanımlayıcı tipte olan bu araştırma, Mayıs 2015-Mayıs 2016 tarihleri arasında Jinekoloji Polikliniğine hirsutizm yakınması ile başvuran 124 kadının dosyası retrospektif olarak incelenerek yapıldı. Hirsutizm varlığı modiye FG skorlaması ile değerlendirildi, FG skor toplamı 8-12 aralığında hafif, 13-18 ise orta, ≥19 olanlarda ağır derecede hirsutizm olarak sınıflandırıldı. Bulgular: Katılımcıların yaş ortalaması 23.96±5.98 (14-41) yıl, FG skoru ortalaması 13.89±4.18 (8-30) puan idi. Kadınların %43.5’inde (n=54) hafif hirsutizm, %44.4’ünde (n=55) orta, %12.1’inde (n=15) ağır hirsutizm tespit edildi. FG skorları ile Beden Kitle İndeksi (BKİ) ve 17-hidroksi progesteron (17-OHP) değerleri karşılaştırıldığında; ağır hirsutizmi olan grupta BKİ ortalama değerleri hafif hirsutizmi olan gruba göre daha yüksekti, en yüksek 17-OHP ortalaması ise ağır hirsutizmi olan gruptaydı. Hastaların %80.6’sında (n=100) PKOS, %16.2’sinde (n=20) idiyopatik hirsutizm, %3.2’sinde (n=4) hiperandrojenik hirsutizm tespit edildi.  Sonuç: Birinci basamakta hirsutizm yakınması ile gelen olgular, FG skoru kullanılarak pratik olarak değerlendirilmesi mümkündür. PKOS gibi etyolojik sebeplerin gözden geçirilmesi önemlidir.

Materials and Methods: This descriptive study was conducted retrospectively between May 2015 and May 2016 in 124 women who visited the Gynecologic Polyclinic with complaints of hirsutism. Hirsutism was assessed by modal FG scoring, FG scores between 8-12 were classified as mild, between 13-18 as moderate, and above 19 as level hirsutism. Results: The average age of the participants was 23.96 ± 5.98 (14-41) years and the FG score was 13.89 ± 4.18 (8-30) points. Mild hirsutism was found in 43.5% of women (n = 54), moderate hirsutism in 44.4% (n = 55) and level hirsutism in 12.1% (n = 15). FG scores and Body Mass Index (BMI) and 17-hydroxyprogesterone (17-OHP) values were compared; In the group with level hirsutism, the average BMI was higher than the group with mild hirsutism and the highest 17-OHP was in the group with level hirsutism. PCOS was found in 80.6% (n = 100), idiopathic hirsutism in 16.2% (n = 20) and hyperandrogenic hirsutism in 3.2% (n = 4) of the patients.  Conclusion: It is possible to evaluate the FG score practically in cases with hirsutism complaints at the primary health care centers. Monitoring of etiological factors such as policystic over syndrome (PCOS) is important.
