Amaç: Bu çalışmada anneleri etkileyen stres faktörlerini saptamak, annelerin Yenidoğan Yoğun Bakım Ünitesi (YYBÜ) çalışanlarından beklentilerini ve bunlarla baş etmede birinci basamak hekiminin rolünü belirlemek planlandı. Metod: Eskişehir Osmangazi Üniversitesi Hastanesinde YYBÜ'de bebekleri izlenen 78 anne ile postpartum 3. gün ve sonrasında yüz yüze görüşülerek sosyodemografik özellikleri, prenatal, natal ve postnatal öyküleri kaydedildi. Annelere Durumluk-Sürekli Kaygı Envanteri ile Genel Sağlık Anketi uygulandı. Bulgular: YYBÜ’ye kabul edilme nedenlerindensık olanı prematüriteydi. Annelerin psikolojik stres düzeylerini etkileyen pek çok erken saptandı. Bebeklerine temas eden ya da onları görmeye giden 41 annenin durumluk-sürekli kaygı düzeyinde artış olduğu, hospitalizasyon süresi arttıkça ve gestasyonel yaş düştükçe kaygı düzeyleri, depresyona eğilimlerinin arttığı saptandı. Düşük doğum ağırlıklı bebeklerin annelerinin kaygı düzeylerini arttırdığı, eğitim düzeyi arttıkça ise depresyon varlığı ve kaygı düzeylerinin düştüğü saptandı. Anne yaşı ile depresyon varlığı ve kaygı düzeyleri arasında ilişki saptanmadı. Düşük riski ya da intrauterin bebek ölümü öyküsü olan annelerin sürekli kaygı düzeyleri arttığı saptandı. Sonuç: Prematür bebek ve konjenital anomalili bebek doğumu, birçok kadının psikoteröpatik tedaviye ihtiyaç duyduğu oldukça travmatik bir olaydır. Türkiye’de aile hekimliği uygulaması başlamıştır. Aile hekimleri yüksek riskli bebek doğumundan sonra anne ve bebeklerin izlemlerini koordine etmekle ve özellikle annelerde bu dönemde artmış olan kaygı düzeyleri nedeniyle aileleri yakından takip etmekle sorumludur
Purpose: In this study, it was planned to identify the stress factors affecting mothers, determine the expectations of mothers from the employees of the Newborn Intensive Care Unit (YYBU) and the role of the first-stage physician in dealing with them. Method: In Eskişehir Osmangazi University Hospital, 78 mothers who followed babies in YYBÜ with postpartum 3rd day and later face-to-face examinations, sociodemographic characteristics, prenatal, natal and postnatal stories were recorded. The mother has been given a general health survey with a state-of-the-counter worries inventory. The conclusions: The reason for acceptance to YYBU was prematurity. A lot of early detected that affected the levels of psychological stress of mothers. It was found that 41 mothers who contacted their babies or went to see them had an increased status-continuous anxiety level, the increased hospitalization time and the decrease of gestational age, the anxiety levels, the tendency to depression increased. It was found that low-born babies increased their mothers’ anxiety levels, while the level of education increased, the presence of depression and the level of anxiety decreased. The relationship between the age of the mother and the presence of depression and the level of anxiety has not been established. It has been found that mothers with a low risk or an intrauterine child death history have constantly increased anxiety levels. The result: premature baby and congenital abnormal baby birth is a quite traumatic event that many women need psychotherapeutic treatment. In Turkey, family medicine has begun. Family doctors are responsible for coordinating the surveillance of mothers and babies after high-risk baby birth and closely monitoring their families due to increased levels of anxiety, especially in mothers during this period.
