Obezite Cerrahisi Sonrası Hastaların Sağlıklı Yaşam Biçimi Davranışları Yeme Tutumları ve Kilo Durumlarının Değerlendirilmesi

Abstract:

Amaç: Bu araştırma obezite cerrahisi sonrası hastaların sağlıklı yaşam biçimi davranışları, yeme tutumları ve kilo durumlarını değerlendirmek amacı ile yapılmıştır. Yöntemler: Tanımlayıcı ve kesitsel nitelikte tasarlanan araştırma, Şubat-Ekim 2019 tarihleri arasında Atatürk Üniversitesi Araştırma Hastanesi’nde yürütülmüştür. Araştırmaya genel cerrahi kliniğine son 5 yılda başvurarak sleeve gastrektomi yapılan toplam 103 hasta alınmıştır. Verilerin toplanmasında Soru Formu, Sağlıklı Yaşam Biçimi Davranışları (SYBD) II Ölçeği ve Üç Faktörlü Beslenme Anketi kullanılmıştır. Verilerin değerlendirilmesinde frekans, ortalama, yüzde, Kruskal Walllis, Mann-Whitney U, one way anova ve independent t testleri kullanılmıştır. Bulgular: Çalışmada hastaların SYBD toplam puan ortalaması 131,64 ± 21,32 olarak belirlenmiştir. Eğitim durumu yüksek olan, ameliyat sonrası kilo vermeye ve egzersize devam eden, BKİ’ne göre normal kiloda olan, diyetisyen desteği alan hastaların; SYBD toplam puan ortalaması, fiziksel aktivite ve beslenme alt boyut puan ortalamaları yüksek bulunmuştur (P < ,05). Ameliyatın üzerinden 1-2 yıl geçen hastaların SYBD toplam puanı ve beslenme alt boyut puan ortalaması; ameliyat sonrası SYBD hakkında bilgilendirilen hastaların SYBD toplam puan ve fiziksel aktivite alt boyut puan ortalamasının yüksek olduğu belirlenmiştir (P < ,05). Eğitim durumu yüksek olan hastaların; bilinçli yemeği kısıtlama puanı, kilo vermesi 12-18 aydan sonra duran hastaların; açlığa duyarlılık alt boyut puanı, sağlık problemi nedeni ile ameliyat olan hastaların; kontrolsüz yeme alt boyut puanları anlamlı olarak yüksek bulunmuştur (P < ,05). Hastaların %40,8’inin ameliyatın 12-18. ayından sonra en düşük kiloya ulaştığı, %41,7’sinin ameliyat sonrası obez seviyesine düştüğü ve %44,7’sinin ameliyat sonrası kilo almadığı saptanmıştır. Sonuç: Obezite cerrahisi sonrası hastaların sağlıklı yaşam biçimi davranışları, yeme tutumları ve kilo durumlarının değerlendirilmesinin önemli olduğu görülmüştür.

Abstract:

Objective: This study was conducted to evaluate healthy lifestyle behaviors, eating attitudes and weight of patients after obesity surgery. Methods: The research, which was designed as descriptive and cross-sectional was carried out at Atatürk University Research Hospital between February and October 2019. The study was completed with a total of 103 patients who applied to the general surgery clinic in the last 5 years and underwent sleeve gastrectomy. Questionnaire, Healthy Lifestyle Behaviors (HLSB) II Scale and Three-Factor Nutrition Questionnaire were used to collect data. Frequency, mean, percentage, Kruskal Walllis, Mann-WhitneyU, one way anova and independent t tests were used to evaluate the data. Results: In the study, it was determined that the mean HLSB scale total score of the patients was 131.64± 21.32. It was found that the mean HLSB scale total score, physical activity and nutrition sub-dimension score averages of the patients with a high education level, who continued to lose weight and exercise after surgery, had a normal weight according to BMI, and received dietitian support were found to be high (P < .05). The mean HLSB scale total score and nutrition sub-dimension score averages of the patients 1-2 years after the operation, and the mean HLSB scale total and physical sub-dimension scores of the patients who were informed about the healthy lifestyle changes after the surgery were found to be significantly higher (P < .05). The conscious food restriction score of the patients with high educational status, the sensitivity to hunger sub-dimension score of the patients whose weight loss stopped after 12-18 months, and the uncontrolled eating sub-dimension score of the patients who had surgery due to health problems were found to be high (P< .05). It was determined that 40.8% of the patients decrease to their lowest weight 12-18 months after the surgery, 41.7% were obese after the surgery and 44.7% did not gain weight after the surgery. Conclusion: It has been observed that it is important to evaluate healthy lifestyle behaviors, eating attitudes and weight status of patients after bariatric surgery.
