Amaç: Bu çalışmada göz kliniğinde uygulanan cerrahi işlemler öncesi alınan aydınlatılmış onam formlarının değerlendirilmesi amaçlanmıştır. Yöntem: Göz kliniğinde cerrahi işlem uygulanmış 300 hastanın aydınlatılmış onam formları, ameliyat türü, onamdaki imzanın hastaya ait olup olmadığı, imza başkasına aitse nedeni, tüm sayfaların onaylanıp onaylanmadığı, hastanın el yazısı ile rıza beyanının varlığı, imzanın kimin tarafından alındığı açısından retrospektif olarak değerlendirildi. Bulgular: Farklı cerrahi işlemler uygulanan 300 hastanın onam formları incelendiğinde imzaların, 281’i hastalara, 19’u ise başkasına aitti. Tüm sayfaların imzalanıp imzalanmadığı değerlendirildiğinde ise, hiçbir onam formunun buna uygun bir şekilde tüm sayfalarının imzalanmadığı, sadece hastanın isim ve imzasının olduğu tek bir sayfanın imzalandığı görüldü. Formlar hastalara, ameliyat günü operasyondan önce sekreter tarafından imzalatılmıştı. Sonuç: Çalışmamızda, aydınlatılmış onam formlarının pek çok açıdan yetersiz olduğu görüldü. Hekimin hem kendini olası bir tıbbi uygulama hatası iddiası riskine karşı koruyabilmesi hem de hastalarının hakkını gözetebilmesi için, aydınlatmanın ve onam almanın etik ve tıbbi usule uygun yapılması önemlidir.
Purpose: This study is aimed at evaluating the lighted forms of confirmation taken prior to the surgical procedures applied in the eye clinic. Method: Illuminated approval forms of 300 patients applied in the eye clinic, the type of surgery, whether the signature in the approval belongs to the patient, the reason if the signature belongs to someone else, whether all the pages were approved or not, the presence of the patient's handwriting and the statement of consent, the signing was considered retrospective in terms of who received the signature. Results: When the approval forms of 300 patients applied different surgical procedures were examined, the signatures were 281 to patients and 19 to others. When it was assessed whether all the pages were signed or not, it was seen that no confirmation form was signed in accordance with all the pages, only a single page with the patient's name and signature. The form was signed by the secretary to the patients on the day of the operation. The result: In our study, it was found that illuminated forms of confirmation were inadequate in many ways. It is important that the doctor can both protect himself against the risk of alleging a possible medical application error and that he can monitor the rights of his patients, that the lighting and approval be made in accordance with the ethical and medical procedure.
