Abstract:

In the context of changing economic policies in the world, health policies and systems are also changing over time. Over the past 15 to 20 years, we have seen a significant change in our health sector. The beginning of patient-centered and evidence-based medical models to provide health services has moved the patient-doctoral relationship beyond the usual. Political statements, developments in patient rights, changes in the Turkish Criminal Law, the implementation of compulsory professional medical insurance and the negative attitude of the press have created unproportionate increases in the remuneration amounts with the medical income. In this environment, the doctors appear to be directed to defensive medical practices with their own protective drive. The research is aimed at a thorough investigation of defense medical practices. The study is carried out by a mixed method where cutting, identifying, quantum and quality approaches are used together. The data was collected by the researcher through the survey and deep interview forms created in accordance with the literature. In the study gradually conducted in June 2018 and January 2019, 253 doctors were surveyed, 30 doctors were thoroughly consulted and 596 patients file in thoroughly consulted were subjected to document examination in the framework of defense practices. Data analysis uses SPSS 13.00, RQ and Excel programs. In the quantitative part of the study, it is shown that the attitudes of doctors towards defense medical practices are high. In the two selected parameters, the attitude score in the laboratory requests was 40.3% of the application rate of 10.6%, while in viewing the attitude score was 37.9% of the application rate of 14.20%. The doctor’s attitude towards defense medical practices is very high, but the high attitude to the practices is still low. In deep discussions it was determined that the accumulation, confusion and patient intensity in front of the clinic directed the doctors to defense medical practices. It is also often expressed that defense medical practices are included as a new treatment protocol in the routine of the medical profession.
