Abstract:

Introduction: Pancreatolithiasis is a rare medical condition and, most of the time, is a sequelae of chronic pancreatitis. The most common etiology is alcoholism and smoking. While its pathogenesis is not yet clear, two hypotheses explain that inflammation and lithostatin deficiency are two of the main cause of stone formation inside the pancreatic duct. The main purpose of treatment is stone extraction and pain improvement. While its recurrence is quite high, surgery as one of the treatments of choice, still gives decent stone-free rate and pain improvement. Case: Male 49 years old with epigastric pain radiating to his back and nausea. Abdominal Magnetic Resonance Imaging showed multiple hypointense pancreatic stone measuring approximately 8 mm with dilatation of the pancreatic duct and distal common bile duct measuring approximately 15 mm and 12 mm. Modified Puestow procedure was performed in this patient. An incision was made starting from the neck to the distal of the pancreas. Conclusion: Pancreatolithiasis is a challenge for diagnosis, accurate diagnosis and open surgery is the definitive therapy in cases of pancreatolithiasis
