KARPAL TÜNEL SENDROMLU HASTALARDA KAS KUVVET VE ENDURANSI ÜST EKSTREMİTE FONKSİYONLARINI ETKİLER Mİ?

Abstract:

Amaç: Karpal Tünel Sendromu (KTS) elde duyusal ve motor kayıplar ile ortaya çıkan bir tuzak nöropatisidir. KTS ile hastaların azalan kas kuvvetleri ve enduransları fonksiyonel aktivitelerde yetersizliklere neden olabilir. Bu çalışma KTS’li hastalarda kas kuvvet ve enduransının üst ekstremite fonksiyonelliği üzerine olan etkisini araştırmak amacı ile planlandı. Yöntem: Çalışmaya KTS tanısı alan 45 hasta (yaş ortalaması 49.44±9.04 yıl) dâhil edildi.  KTS’li hastaların üst ekstremite kas enduransı, desteksiz kol testi (1. ve 2. diagonal paternde)  ile, üst ekstremite kas kuvveti Hand Held Dinamometre ile, kavrama kuvvetleri hand dinamometre, parmak kavrama kuvveti digital pinçmetre ile, üst ekstremite fonksiyonelliğini değerlendirmek için ise DASH (Kol, Omuz ve El Yaralanmaları Anketi) kullanıldı. Sonuçlar: Hastaların DASH skorları ile üst ekstremite omuz fleksör, ekstansör, abduktör, eksternal rotatör, dirsek fleksör-ekstansör, el bileği ekstansör ve tüm pinch kuvvetleri arasında negatif yönde anlamlı ilişki olduğu belirlendi (p<0,05). Hastaların DASH skorları ile üst ekstremite desteksiz kol testi (1. ve 2. diagonal paternde) sonuçları arasında negatif yönde anlamlı ilişki olduğu bulundu (p<0,05). Tartışma: Karpal tünel sendromlu hastalarda özellikle azalmış kassal endurans ve kassal kuvvet üst ekstremite fonksiyonlarında azalmalara neden olmaktadır. Karpal tünel sendromlu hastalarda ağrı ve benzeri semptomlar ile azalan üst ekstremite fonksiyonelliğini arttırmak için kassal kuvvetin yanısıra uzun dönemde kassal endurans egzersizlerinin de fizyoterapi programlarına dahil edilmesi gerektiğini düşünmekteyiz. Does the carpal tunnel syndrome affect the function of the upper extremity in patients?

Target: Carpal Tunnel Syndrome (KTS) is a trap neuropathy that occurs with sensual and motor losses. Reduced muscle strength and resistance of patients with KTS can cause disadvantages in functional activities. This study was planned to investigate the effects of muscle strength and endurance on the functionality of the upper ends in patients with KTS. Method: The study included 45 patients diagnosed with KTS (the average age is 49.44±9.04 years).  Upper end muscle endurance of patients with KTS, unsupported arms test (1 and 2). With the diagonal paternde, the upper extremity muscle force with the Hand Held Dynamometer, the capture forces with the hand dynamometer, the finger capture force with the digital pinchmeter, and the upper extremity functionality was used by DASH (Kol, Arm and Hand Injury Survey). Results: Patients’ DASH scores have a meaningful relationship in the negative direction between the upper extremity shoulder flexor, extender, abductor, external rotor, rear flexor-extender, handheld extender and all pinch forces (p<0,05). Patients found a significant link in the negative direction between the DASH scores and the outcome of the upper extremity unsupported arm test (1 and 2 diagonal paternde) (p<0,05). Discussions: In patients with Carpal Tunnel Syndrome, a decrease in casal endurance and casal strength causes decrease in the upper end functions. In patients with carpal tunnel syndrome, we consider that in order to increase the functionality of the upper end with pain and similar symptoms decreased, as well as kassal strength, long-term kassal endurance exercises should also be included in the physiotherapy programs.
