Amaç: Bu çalışmada kliniğimizde obturator herni nedeni ile opere edilen hastaların tanı, tedavi seçenekleri ve hasta yönetiminin literatür ile karşılaştırılarak incelenmesi amaçlanmıştır Gereç ve Yöntem: Ocak 2009-Eylül 2013 tarihleri arasında Ege Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalı’nda akut karın ve mekanik barsak tıkanıklığı nedeni ile opere edilen ve etyolojik neden olarak preoperatif tanı konularak acil olarak opere edilen veya peroperatif obturator herni tespit edilen 4 hasta retrospektif olarak incelendi. Bulgular: Obturator herni nedeni ile opere edilen 4 hasta retrospektif olarak incelendi. Hastalarda radyolojik olarak abdominal bilgisayarlı tomografide mekanik barsak tıkanıklığı ile uyumlu patolojiler ve direk karın grafisinde hava-sıvı seviyeleri olduğu tespit edildi. Obturator herniye bağlı inkarserasyon veya strangülasyon nedeni ile opere edilen hastaların 2 tanesine (%50) parsiyel ince barsak rezeksiyonu ve anastomoz uygulanırken, 2 hastaya (%50) redüksiyon uygulandı. Postop takipte 1 hastada (%25) derin ven trombozu gelişirken, 1 hasta (%25) anastomoz kaçağı gelişmesi sonrasında exitus oldu. Sonuç: Obturator herni nadir görülen bir herniasyon çeşididir. Sıklıkla ileri yaş bayanlarda görülür. Tanıda gecikme mortalite ile sonuçlanabildiği gibi postoperatif dönemde de ileri yaşa bağlı komplikasyonlar (derin ven trombozu, pnomoni) her zaman göz önünde bulundurulmalıdır. Mekanik barsak obstruksiyonu ayırıcı tanısında obturator herni akılda tutulmalı tanı ve tedavide gecikilmemelidir. Purpose: This study in our clinic is intended to study the diagnosis, treatment options and patient management in comparison with the literature of patients operated with the cause of obturator herni Method: January 2009-September 2013 by the University of Ege Medical School General Surgery Anabolic Department operated with the cause of acute abdominal and mechanical abdominal congestive and the ethological cause of the preoperative diagnosis was urgently operated or peroperative obturator herni was identified as 4 patients retrospective examined. Results: 4 patients operated with the cause of Obturator herni were examined retrospectively. In patients radiologically abdominal computer tomography was detected that there were pathologies compatible with mechanical abdominal blocks and air-fluid levels in the direct abdominal graph. 2 of the patients operated due to inkarseration or strangulation associated with obturator hernias (50%) were given partial thin abdominal resection and anastomosis, while 2 patients (50%) were given reduction. Postop follows 1 patient (25%) develops deep venous thrombosis, while 1 patient (25%) has exitus after anastomosis escape development. The result: Obturator herni is a rare type of herniation. It is often seen in older women. Diagnosis delays can result in mortality as well as in postoperative period, advanced age-related complications (deep vein thrombosis, pneumonia) should always be considered. In the distinctive diagnosis of mechanical abdominal obstruction, the diagnosis and treatment of obturator herni should not be delayed.
