Abstract:

The differential block is defined as a separate inhibition of the sensory and motor activity of the nerve. Differential peripheral nerve blocks are aimed at preserving motor activity and inhibiting sensory activity. This makes it possible to study the movements of tendons and muscles intraoperatively. This study aimed at the retrospective assessment of patients with differential upper end block accompanied by ultrasound we conducted in our clinic and presenting our experiences.   Instruments and Methods: In the period of 2 years between 2016 and 2018, anesthesia records of patients with a brakial plexus block of infraclavics, accompanied by 0.5 % prilokain and differential block ultrasound, were evaluated. Demographic data of patients, block application indications, block start time, duration of the operation, the presence of intraoperative-postoperative complications were studied. Emotional block, between 0 and 2, motor block, was assessed using a numerical scale between 0 and 5.   Results: 43 patients (24 women, 19 men) were included in the study. The average operating time was 47.5 ± 20.71 minutes. An average of 19,53 ± 3,04 ml of 0.5 % prilocain was used to obtain differential brakial plexus block. The medium engine block score was 3 (1-5) . In 41 cases the engine function was sufficient, in 2 cases the engine block score was 1 and the peroperative engine function was not sufficient.  No allergic reaction, intraoperative and postoperative complications were developed. Result: In our clinic, the differential brakial plexus block with 0.5 % prilokain was successfully performed in the upper limb soft tissue operations. There are no side effects of Prilokaine.
