Amaç: Bu çalışmada, ince barsak obstrüksiyonlarının etiyoloji ve cerrahi sonuçları açısından incelenmesi amaçlandı. Gereç ve Yöntem: Merkezimizde Ocak 2001 ile Eylül 2015 tarihleri arasında ileus nedeniyle başvuran hastalar retrospektif kesitsel olarak incelendi. Nadir mekanik ince barsak obstrüksiyonu olan hastalar dahil edildi. Hastalar; demografik özellikleri, etiyoloji, etiyolojiye yatkınlık oluşturan durumlar, tanı çalışmaları, tanı çalışmalarının başarısı, tedavi yöntemleri ve sonuçları açısından incelendi. Bulgular: Belirtilen tarihler arasında 481 hasta mekanik ince barsak obstrüksiyonu nedeniyle ameliyat edildi. Hastaların 13’ünde (%2,8) diğer nedenlere bağlı obstrüksiyon saptandı. Bu hastaların 5’inde bezoar (%1,0), 2’sinde B hücreli lenfoma (%0,4), 1’inde yabancı cisim (%0,2); 3’ünde safra taşı (%0,6), 1’inde gastrointestinal stromal tümör (%0,2) ve bir hastada endometriyozise (%0,2) bağlı ince barsak obstrüksiyon mevcuttu. Cerrahi tedavide ezerek ve sağılarak bezoarın ilerletilmesi, enterotomi, enterolitotomi, segmenter ince barsak rezeksiyonu ve ileoçekal rezeksiyon uygulandı. Ameliyat sonrası tek komplikasyon, bir hastada (%7,69) görülen yüzeyel yara yeri enfeksiyonu idi. İzlem süresinde, 3 hastada (%23,0) insizyonel herni görüldü. Hastaların hiçbirinde ince barsak tıkanıklığı nedeniyle yeniden ameliyat gerekliliği olmadı. Sonuç: İnce barsak obstrüksiyonuna yol açan nadir nedenlerin ameliyat öncesi tanı konulması zordur. Adezyon riski olmayan ince barsak obstrüksiyonlu hastalarda bu nadir etiyolojiler de göz önünde tutularak erken dönemde cerrahi tedavi düşünülmelidir.
Purpose: In this study, the aim was to study thin abdominal obstruction from the point of view of the etiological and surgical results. Method and means: Patients who apply for withdrawal from January 2001 to September 2015 in our center were examined retrospectively. Patients with rare mechanical thin abdominal obstruction were included. Patients have been studied in terms of their demographic characteristics, etiology, conditions that form the tendency to etiology, diagnostic studies, the success of diagnostic studies, therapeutic methods and results. Results: Between the specified dates, 481 patients were operated due to mechanical thin abdominal obstruction. In 13 patients (2.8 percent) obstruction was detected due to other causes. In 5 of these patients, bezoar (1.0%) and 2 in B cell lymphoma (0.4%) and 1 in foreign bodies (0.2%) and 3 in gallstone (0.6%) and 1 in gastrointestinal stromal tumor (0.2%) and a patient with endometriosis (0.2%) had thin abdominal obstruction related to endometriosis. In the surgical treatment, the progression of the bladder was applied by cutting and rectum, enterotomy, enterolithotomy, segmentary thin abdominal resection, and iosecal resection. The only complication after surgery was the surface wound site infection that was seen in a patient (7.69%). During the monitoring period, 3 patients (23,0%) had insuficient herni. None of the patients had no need for re-operation due to thin abdominal blocks. The result: rare causes that lead to thin abdominal obstruction are difficult to diagnose before surgery. In patients with thin abdominal obstruction without the risk of adhesion, this rare etiology should also be taken into account in the early period of surgery.
