Amaç: Diabetes Mellitus (DM); İnsülin hormon sekresyonu ve insülin etkisinin azlığı sonucu karbonhidrat, protein ve yağ metabolizmasında bozukluklara yol açan kronik metabolizma hastalığıdır. Sirtuin1 (SIRT-1) geni, glukoneogenez geninin indüksiyonunda ve glikolitik genin regresyonunda gereklidir. Yapılan çalışmalara göre SIRT-1 geni iskelet kasında insülin duyarlılığının geliştirilmesinde önemli bir rol oynar. Bu çalışmada SIRT-1 Gen polimorfizmi ve ekspresyonu ile Tip 2 DM hastalığına yatkınlık, tedavide etkinliği ve kontrol arasındaki ilişki araştırılmıştır. Materyal ve Metod: Bu çalışma Elazığ ili civarındaki Tip 2 DM tanısı almış 30 yaş üzeri 40 kişi ve diyabet tanısı olmayan 40 kişilik kontrol grubunda yapıldı. Çalışma gruplarında rutin kan tetkiklerine ek olarak Sirtuin1 (SIRT-1) gen polimorfizmi ve gen ekspresyonu çalışıldı. Bulgular: Çalışmamızda Tip 2 DM (n=40) ve kontrol (n=40) grupları SIRT-1 rs7895833 gen bölgesi polimorfizmleri açısından karşılaştırıldı. SIRT-1 rs7895833 gen bölgesi polimorfizimi oranları açısından bu iki grup arasında anlamlı bir fark yoktu (p=0.383). SIRT-1 rs7895833 Single Nükleotid Polimorfizmi ile BMI arasında anlamlı ilişki tespit edilmiştir.(p=0.01-p=0.04). Sonuç: Çalışmamızda SIRT-1 rs7895833 Single Nükleotid Polimorfizmi bölgeleri açısından Tip 2 DM ve kontrol grubu arasında anlamlı farklılık saptanmamıştır. Fakat her iki gen expresyonu ile Vücut Kitle İndeksi (VKİ) arasında anlamlı ilişki tespit edilmiştir. Yapılan fonksiyonel çalışmaların yanında SIRT-1'in genetik ve farmakolojik olarak inhibisyonunun insülin direncini indüklemesi gelecekteki yeni nesil farmasötiklerin SIRT-1 ekspresyonunu negatif veya pozitif olarak etkileyen yolakların hedef olarak kullanılacaklarını göstermektedir.
Diabetes Mellitus (DM) is a chronic metabolism disease that causes disorders in the metabolism of carbohydrates, proteins and fats as a result of insulin hormone secretion and insulin deficiency. Sirtuin1 (SIRT-1) gene is necessary in the induction of the gluconeogenesis gene and in the regression of the glycolytic gene. According to studies, the SIRT-1 gene plays an important role in developing insulin sensitivity in the skeletal muscle. This study examined the relationship between SIRT-1 gen polymorphism and expression and the tendency to type 2 DM disease, the effectiveness of treatment and control. Material and Method: This study was conducted in a control group of 40 people over the age of 30 who were diagnosed with Type 2 DM and 40 who were not diagnosed with diabetes. In addition to routine blood tests in the study groups, gen polymorphism and gen expression were studied in Sirtuin1 (SIRT-1). Results: In our study, the type 2 DM (n=40) and control (n=40) groups were compared in terms of the polymorphisms of the SIRT-1 rs7895833 gen area. There was no significant difference between these two groups in terms of the polymorphism rates of the gene area SIRT-1 rs7895833 (p=0.383). SIRT-1 rs7895833 Single Nucleotide Polymorphism and BMI have a significant relationship (p=0.01-p=0.04). In our study, SIRT-1 rs7895833 has not found any significant differences between Type 2 DM and control group in terms of single nucleotid polymorphism areas. However, a meaningful relationship between both genes and the body mass index has been found. In addition to functional studies, the inhibition of the genetic and pharmacological inhibition of SIRT-1 inducing insulin resistance shows that the next generation of pharmacists will be used as the target of the pathways that negatively or positively affect the SIRT-1 expression.
