Morbid Obezite Cerrahisi Ve Komplikasyonlar

Abstract:

Obezite ameliyatları, morbid obez hastalarda kilo kaybının devamlılığının sağlanmasında oldukça etkili yöntemdir. Obezite ameliyatlarından sonra ameliyatın tipine özel komplikasyonlar olmasına karşın bunların dışında erken ve geç dönem komplikasyonlar da görülmektedir. Çalışmamızda, morbid obezite nedeniyle bariartrik cerrahi uyguladığımız hastaların ameliyat esnasında veya ameliyat sonrasında tespit edilen komplikasyonlarını sunmayı amaçladık. Toplam 361 hastaya bariatrik cerrahi uygulandı. Hastaların 207’sine (%57.3) laparoskopik sleeve gastrektomi (LSG), 38’ine (%10.5) laparoskopik Roux-n-Y gastrik bypass (LRYGB), 32’sine (%8.8) Roux-n-Y gastrik bypass (RYGB), 32’sine (%8.8) mini gastrik bypass (MGB), 28’ine (%7.7) laparoskopik mini gastrik bypass (LMGB), 21’ine (%5.8) sleeve gastrektomi (SG), 2’sine (%0.5) vertikal bantlı gastroplasti (VBG) ve 1’ine (%0.2) intragastrik balon yerleştirilmesi işlemleri yapıldı. Roux-n-Y gastrik bypass yapılan 2 hastada (%0.5) anastomoz kaçağı tespit edildi. SG yapılan bir hastada stapler hattından kaçak meydana geldi. 1 hastada evisserasyon, 1 hastada ameliyat esnasında dalak yaralanması, 44 hastada (%12.1) cerrahi alan enfeksiyonu tespit edildi. Cerrahi alan enfeksiyonu tespit edilen hastaların %91’inde diabetes mellitus mevcuttu. Laparoskopik bariatrik cerrahi yapılan hastalarda cerrahi alan enfeksiyonu oranı %6.8 iken açık cerrahide bu oran %29.4’e kadar çıktığı görüldü. 9 hastada (%2.4) ileus, 3 hastada (%0.8) pulmoner emboli, 8 hastada (%2.2) DVT gelişti. Süper obez 2 hastada postoperatif akciğer yetmezliğine bağlı mortalite meydana geldi. Bariatrik cerrahide hangi ameliyat tekniği tercih edilirse edilsin morbidite ile karşılaşılması muhtemeldir. Bu nedenle morbid obez hasta seçiminde ve tedavi sürecinde komplikasyon açısından dikkatli olunması gerekmektedir.

Obesity surgery is a very effective way to ensure the continuity of weight loss in patients with morbid obesity. Although there are special complications for the type of surgery after obesity surgery, in addition to these, early and late period complications are also seen. In our study, we aimed at presenting the complications identified during or after surgery of patients we undergo bariarthric surgery due to morbid obesity. A total of 361 patients were undergoing bariatric surgery. 207 patients (57.3%) laparoscopic sleeve gastrectomy (LSG), 38 (10.5%) laparoscopic Roux-n-Y gastric bypass (LRYGB), 32 (8.8%) Roux-n-Y gastric bypass (RYGB), 32 (8.8%) mini gastric bypass (MGB), 28 (7.7%) laparoscopic mini gastric bypass (LMGB), 21 (5%). 8) sleeve gastrectomy (SG), 2 (%0.5) vertical band gastroplasty (VBG) and 1 (%0.2) intragastric balloon placement processes were performed. In 2 patients with Roux-n-Y gastrointestinal bypass (0.5%) anastomosis was detected. A patient with SG was caught from the stapler line. 1 patient was diagnosed with evaporation, 1 patient was injured during surgery, 44 patients (12.1%) were diagnosed with an infection in the surgical area. In 91% of patients diagnosed with an infection in the surgical area, diabetes mellitus was present. In patients with laparoscopic bariatric surgery, the infection rate in the area of surgery was 6.8%, while in open surgery this rate was up to 29.4%. In 9 patients (2.4%) Ileus, in 3 patients (0.8%) pulmonary embolism, in 8 patients (2.2%) DVT developed. Super obesity in 2 patients occurred mortality due to postoperative lungs failure. In the Bariatric Surgery, which surgery technique is preferred, it is likely that you will face morbidity. Therefore, morbid obesity patients should be cautious in terms of complications in the selection and treatment process.
