Defansif uygulamalar, hekimlerin kendilerini korumak için gösterdikleri kaçınma davranışlarıdır. Bu uygulamaların dünyada olduğu gibi Türkiye’de de hekimler tarafından yaygın olarak uygulandığı görülmektedir Bu çalışmada defansif tıp uygulamalarının nedenleri ve yaygınlığına ilişkin değerlendirme yapmak amaçlanmıştır. Defansif uygulamalar kaçınma davranışı olarak değerlendirilen negatif defansif tıp ve daha çok girişime yönlenilen pozitif defansif tıp olma üzere iki başlık altında toplanmaktadır. Her iki uygulamada da hekimleri defansif tıbba yönlendiren nedenler bulunmaktadır. Bunlar hasta-hekim iletişimi, medyanın etkisi, şiddet, hekimlerin iş yükünün artması, malpraktis davaları, sağlık sistemi, mesleki deneyimin az olması, şikâyet edilme korkusu olarak sıralanmaktır. Sonuç olarak defansif uygulamalar, hekimler ve hastalar açısından olumsuz durumlara sebep olmaktadır. Zira sağlık hizmetlerinin etkili sunumunun sağlanması için hekimleri bu uygulamalara yönelten sebeplerin irdelenmesi, sorunların çözümlenmesi gerekmektedir. Dolayısıyla defansif tıbbı önlemek için başta yöneticiler olmak üzere sağlık çalışanları ile birlikte çözümler aranmalıdır.
Defense practices are the preventive behaviors that doctors show to protect themselves. These practices appear to be widely applied by doctors in the world as well as in Turkey This study aims to assess the causes and prevalence of defensive medical practices. Defence practices are gathered under two titles to be negative defence medicine, which is considered a preventive behavior, and more initiative-oriented positive defence medicine. In both applications there are reasons that guide the doctors to defensive medical treatment. These are the patient-doctoral communication, the influence of the media, the violence, the increased labor burden of doctors, the cases of malpracticism, the health system, the lack of professional experience, the fear of complaint. As a result, defensive practices cause negative conditions for doctors and patients. In order to ensure the effective presentation of health services, the doctors need to identify the causes of these practices, the problems must be solved. Therefore, in order to prevent defensive medicine, solutions should be sought together with healthcare professionals, primarily managers.
Defensive practices are the avoidance behaviours that physicians show to protect themselves. It is seen that these applications are widely practiced by the physicians in Turkey similar to the cade in the world. In the study, it is aimed to evaluate the reasons and the prevalence of defensive medicine practices. Defensive practices are classified under two headings: negative defensive medicine, which is evaluated as avoidance behaviour, and positive defensive medicine, which is more introverted. In both applications, there are reasons why physicians are leading defensive medicine. These are listed as patient-doctor communication, media effect, violence, the increase in the work load of physicians, malpractice cases, health system, low professional experience and fear of complaint. As a result, defensive practices cause adverse situations in terms of physicians and patients. In order to render effective health services, the causes that lead physicians to these applications should be addressed and the problems should be solved. Therefore, in order to prevent defensive medicine, solutions should be sought together with health professionals, particularly with the managers.
