Konsültasyon, bir hastanın durumu hakkında doktorlar arasında bilgi ve tavsiye alışverişi olarak tanımlanabilir ve sağlık hizmetlerinin önemli bir bileşenidir. Bir taraftan sağlık hizmetlerinin güvenilirliğini ve etkinliğini yükseltirken; diğer taraftan da, hizmetlerinin karmaşıklığını artırmakta, hizmetlerde kesintilere neden olmakta ve zaman gerektirmektedir. Bu nedenle bazı hedeflenmeyen ve istenmeyen etkiler de yaratmaktadır. Bilgi ve iletişim teknolojilerinin (BİT) yükselişiyle, bu teknolojiler, sağlık sistemlerini geliştirmek, sağlık sistemlerinin verimliliğini ve izlenebilirliğini artırmak için neredeyse tüm sağlık hizmeti süreçlerine entegre edilmiştir. BİT'in konsültasyon sürecinde kullanılması, konsültasyonun belirtilen olumsuz etkilerini çözmeyi amaçlamaktadır. Bu çalışmanın amacı hastanede çalışan hekimlerin Hastane Bilgi Yönetim Sistemini (HBYS) kullanarak gerçekleştirdikleri konsültasyon sürecini incelemek ve HBYS’nin konsültasyon sürecine etkilerini değerlendirmektir. Bu çalışmada, nicel ve nitel yaklaşımlar benimsenmiştir. Uygulama bir devlet hastanesinde gerçekleştirilmiştir. Nicel yaklaşımla hastanenin bir yıllık elektronik konsültasyon talepleri (EKT) ele alınmış, nitel yaklaşımla da konsültasyon uygulaması yapan hekimlerle gerçekleştirilen yarı yapılandırılmış mülakatlarla hekim görüşleri derlenmiştir. Çalışmanın sonuçlarına göre genel anlamda bilgi sitemlerinin konsültasyon isteminde kullanılması, hekimlerce olumlu olarak değerlendirilmiştir. Bu sonuçlar hastanenin EKT kullanımı ile konsültasyon için formal bir süreç oluşturduğunu, süreç izlenebilirliğinin arttığını ve konsültasyon süreci üzerindeki kontrolünün geliştiğini göstermektedir. El yazısı okuma zorluğu, kayıp doküman gibi kağıda dayalı sistemler ile ilgili sorunlar ortadan kalkmıştır. Hastanın yerinde olmaması, sistemin yavaş işlemesi, klavyelerin farklı olması gibi alt yapıdan kaynaklanan sorunların süreci olumsuz etkilediği sonucuna ulaşılmıştır. Hastane yönetimi, altyapı sorunlarının üstesinden gelmek için altyapı yatırımı yapabilir. Tasarım problemleri, kullanıcı deneyimini tasarım ve geliştirme süreçlerine dahil ederek çözülebilir.
Consultation can be defined as the exchange of information and advice between doctors about the condition of a patient and is an important component of health care. On the one hand, it increases the reliability and effectiveness of health services; on the other hand, it increases the complexity of its services, causes interruptions in services and takes time. Therefore, it also creates some undesirable and unwanted effects. With the rise of information and communication technologies (ITT), these technologies have been integrated into almost all healthcare processes to improve health systems, improve the efficiency and traceability of health systems. The use of BIT in the consultation process aims to solve the indicated negative effects of the consultation. The aim of this study is to examine the consultation process of doctors working in the hospital using the Hospital Information Management System (HBYS) and to evaluate the effects of HBYS on the consultation process. In this study, qualitative and qualitative approaches have been adopted. The operation was carried out in a state hospital. With the Nicel approach, the hospital’s one-year electronic consultation requests (ECT) have been addressed, and with the quality approach, the doctor’s opinions have been collected with semi-configured interviews conducted with doctors who apply consultation. According to the results of the study, the use of information sites in the general sense in the consultation request was positively assessed by doctors. These findings show that the hospital has formed a formal process for consultation with the use of ECT, the process traceability has increased and the control over the consultation process has developed. Difficulty reading handwriting, problems with paper-based systems such as lost documents have disappeared. The patient’s absence in place, the system’s slow processing, the keyboards are different, and the problems caused by the substructure have a negative impact on the process. Hospital management can make infrastructure investments to overcome infrastructure problems. Design problems can be solved by incorporating user experience into the design and development processes.
