How often do we think of neuroleptic malignant syndrome (NMS) in patients who come to the emergency department with sepsis clinic? Due to its similarity to some organic and psychiatric disorders, this syndrome may be overlooked or difficult to diagnose. Early diagnosis and treatment gain importance in NMS and mortality can be decreased even in severe cases. In this article, we present a case of NMS that developed in a patient diagnosed with bipolar affective disorder, who had been receiving 37.5 mg / 15 days of risperidone, 300 mg / day lithium carbonate maintenance treatment for 8 years, and 2.5 mg / day olanzapine was added to his treatment 1 month ago.
